This skill is causing full paralysis every use, not just after 2 or more uses.

true but it says after 2 or more you only got hit twice i don't see any problem

I've tried to recreate this 6 times and I see no problem.

I've worked out that there's a specific pattern that needs to be done to recreate it, a pattern in the battle log itself. First turn you have to use Hitsugaya's Bankai, and then on your second turn you have to use Byakuya's Massacre Scene Bankai, followed by Hyoten Hyakkaso, which will cause the full paralysis effect.
